Your Health Savings Account (HSA) are tax-advantaged healthcare savings account meaning that it can give you multiple tax savings and advantages. We'll help provide the tax forms you need to file your taxes and take advantage of your HSA tax benefits.1

Your HSA tax forms

HealthEquity provides your HSA tax forms through mail or email, and they are easily accessible online. If you have a Health Reimbursement Arrangement (HRA), healthcare Flexible Spending Account (FSA), or dependent care FSA, you will not receive tax forms from HealthEquity.

You can access your tax forms by logging in to your HealthEquity account and navigating to the Documents section.

Below you will find information about the tax forms associated with your HSA, what to do with them, and how to file them.

Form 1099

Basic information HealthEquity will send you a Form 1099-SA if you had any distributions from your HSA during the tax year.

Instructions Enter the distributions2 shown on your 1099-SA on Form 8889, and file it with your tax return.

Form 5498

Basic information

Form 5498-SA is used for informational purposes only. It reports regular and rollover contributions on HSAs, Archer Medical Savings Accounts (MSAs), and Medicare Advantage MSAs. It is issued by your HSA custodian.

Please note that this form is not typically available until after Tax Day (generally May or June) because contributions for the prior year can be made through Tax Day.

Instructions

Form 5498-SA is for your records only. You do not need to file it with your tax return.3 Verify that the contributions shown match your records and your Form 8889.

Form 8889

Basic information

File Form 8889 with your tax return to report your HSA contributions and distributions. Completing Form 8889 requires information from your 1099-SA and information from your W-2.

This form is available on the IRS website.

Instructions

Complete Form 8889 and attach it to your Form 1040. Report all HSA contributions and distributions, and calculate your HSA deduction.

Form W-2

Basic information

Your W-2 shows your HSA contributions made by your employer and by pre-tax payroll deduction, if applicable.

If your HSA payroll deductions were taken pre-tax, they're already excluded from the wages shown in Box 1 of your W-2, so you don't need to deduct them again.

Return to content 1HSAs are never taxed at a federal income tax level when used appropriately to pay for qualified medical expenses. Also, most states recognize HSA funds as tax-deductible with few exceptions. Please consult a tax advisor regarding your state's specific rules.

Return to content 2Distributions for qualified medical expenses are tax-free. Distributions for non-qualified expenses are subject to income tax and may be subject to a 20% penalty.
