You can designate any individual as a beneficiary for your health savings account (HSA). If you have an HSA balance upon passing away, your beneficiaries will receive the funds.
When adding a beneficiary, you may:
Set what percentage of your total balance should go to each beneficiary.
Name both primary and contingent beneficiaries. Contingent beneficiaries receive funds only if all primary beneficiaries are deceased when you pass away.
Add a trust or estate instead of an individual.
How to submit your request
Click on the 'Complete Online' link above.
Select your marital status.
If you are married and designating a beneficiary who is not your spouse, or if you live in a community property state, an additional signed form is required. The page will provide details for how to submit this form.
Select 'Add Beneficiary.'
Enter personal information for the new beneficiary and click 'OK.'
First name, last name and relationship are required.
Birth date, SSN and address are not required on the form, but must be completed before any funds can be sent to the beneficiary.
Set the Primary % or Contingent %. Primary designations must add up to 100% before you can save your changes or set any contingent designations.
Click 'Save.'
Repeat these steps to add multiple beneficiaries.
